Watch out for the Stranger ‘honor’ missions that also contribute to the completion list. It’s easy to miss those when you’re only looking at the yellow dots. 

Yes, that happens when someone kills you -you get robbed, and any non-legendary pelts are also taken. 
Thankfully, they won’t take all of your money and items - I usually have more than $1000 on me and lose some when getting killed. It doesn’t take much time to get that money back from stealing or selling stuff.
