Guy falling off his horse?

I've seen this a couple times while riding around out in the wild.  I'll be riding down the road and a guy will be riding towards me, i see the white dot on him for a stranger, but before i can do anything the guy is gagging and falls off his horse dead.  I get a little loot from him, maybe a hat if i'm not wearing one. 

What are we supposed to do with this guy?  just watch him gag and fall off his horse?

I've had this happen a couple times , once a guy had arrows in his back and he did the same thing.  Also when i looted the first guy i got 4 gold teeth but idk if there's anything to this or what...

Sometime you can pick these guys up before they die, where you can take them to the doctors. I had to watch while one guy had his arm sawn off. 

Sometines they die before you gamer to them. 

Sometines you trample them to death while getting off your horse.

Huh, guess my reaction time is just too slow.  I'll have to pay closer attention as i'm riding around.  By the time i get to them they're always dead in the road lol

I had this one random encounter in the woods with someone calling for help. He was in really bad shape. He was asking for booze. I had the choice to give him medicine or booze. I gave him medicine. He said "It's too late for that.". He then died.

It’s the guys with snake bites you need to watch out for.  If you get the wrong camera angle while Arthur is sucking the venom out, it looks... well.... wrong.

I've saved all of those i've seen.  gave them the healing tonic each time.  haven't yet tried sucking out the poison.  Got a free gun and a free hat for it so far.  Haven't run into the guy i saved the 3rd time (same guy as the 2nd time) to see if he'll let me put anything else on his tab at a merchant.

I saw this guy once and it was right before (SPOILER ALERT) Arthur gets tuberculosis and I for a moment thought it was the cause before finding the real cause online
