You can check your defensive / offensive status in the Player Menu. I believe it's the last menu option at the bottom that will bring you to the screen where it will show your current state and give you the option to change it. As for how all this is supposed to work, this is it in a nutshell although a couple of things had changed like the way the game is supposed to remember your last setting on the hostility mode although IME, it is always wise to check when you start a new session, etc. to make sure you;re in Defensive mode if that's your preferred setting.

Yeah I do both depending on Jims outfit:- When you are putting items like shirts & trousers on, check the bottom right of the screen for a MODIFY option - this allows you to tweak boots & shirt sleeves & collars - this is the same in campaign & Online Gunfighter - Shirtsleeves rolled up & Vest no coat, Boots under Trousers with spurs The Van Cleef outfit - likewise, boots with spurs under trousers - Coat & Vest on Hunter & Cowpoke outfits - boots outside trousers ready for sloshing through blood, snot, rivers & innards

The pvp is this game is an after thought. They have put zero focus or effort in PVP. Its just sort of there... As far as fashion goes. IMO R* has already went waaaay over board with it. Theres to much. Also the pink and bright colors. Females, in the wild west did not all wear white. Theres plenty of images online of wild west females. The wear a lot of black.
